Our Outreach program provides community support to people with multiple disabilities to maximize their independence and quality of life.
Our Outreach program empowers people with multiple disabilities to grow their independence through tailored skills training and support with appointments, shopping and everything in between. The intent of our program is to help the people who use our services build confidence, increase comfort with new resources to encourage true community inclusion. Our outreach program is completely customizable—we will work with you to develop the best support plan to meet your unique needs. We also offer in-home day respite for caregivers.
Our Outreach program is based on a fee-for-service model which means there are no waiting lists. If you aren’t sure what funding might be available to you, don’t hesitate to reach out. There are many options and if we can’t help we will point you to the right person.
